The immigration medical exam, using form I-693, Report of Immigration Medical Exam and Examination, is a required step in the green card application process. While it might not get as much attention as the evidence or interview, your exam plays just as important of a role in your immigration process.
At KW Law, we’ve worked with countless families through the whole process, including this step, and we know how confusing and nerve-racking it can feel when you’re unsure where to start. Most important to know, the exam MUST be performed by a doctor specifically approved by U.S. Citizenship and Immigration Services (USCIS) called a CIvil Surgeon. Simply stated, your application will be DENIED if it is not performed and reported by this physician.
Why is the Immigration Medical Exam Required?
This exam is meant to confirm that you meet the health standards the United States has set for new permanent residents. It’s overseen by the Centers for Disease Control and Prevention and checks for things like communicable illnesses, missing vaccines, and any medical conditions that might affect your admissibility.
Finding a USCIS Doctor
Fortunately, finding an approved Civil Surgeon is fairly simple. The easiest way to find one is to use the official USCIS search tool here: uscis.gov/tools/find-a-civil-surgeon. Using their search feature, provide your zip code and a list of approved doctors in your area will appear! You can filter your search by preferences of physician by language or even gender to help you find someone you are most comfortable with. After setting your preferences, click the search button to generate your results. We suggest printing your list of these doctors.
How to Know You’re Choosing the Right Doctor
We find our clients are most satisfied with their choice of doctor after having called several offices and asking baseline questions. For example, the government does not determine the how much a provider can (or can’t) charge for the exam, so prices can vary significantly between clinics…. We’ve had clients tell us they’ve paid as little as $150.00 and as much as $500.00. When calling, simply ask “How much do you charge for the immigration medical exam and can my health insurance be applied?”
In addition to cost, exam availability can vary. Some doctors offer same-day appointments while others may be booked weeks in advance so depending on urgency, you may need to make many calls to find someone that will work with your schedule. Many of our clients have shared tha on the call clinic staff will explain what the exam involves, what to bring with you and what to do if you’re missing records or documents. How helpful and responsive the staff is when you speak with them will tell you a lot about that clinic and the likelihood of a smooth examination. Again – never schedule this exam with a doctor not listed on the USCIS site. Even if they’re qualified to practice medicine in other ways, your exam will be nullified.
Preparing for the Exam Appointment
Before your appointment, gather everything you’ll need early so you aren’t left scrambling on the day of. That includes a photo ID (like a passport or driver’s license), your vaccination history, and your health insurance card if you have one. If you’re under 14, make sure your documents include your name, birth date, and a parent’s full name. The clinic will have form I-693 ready for the doctor, but if they indicate you need to bring the blank form with you, please let your KW team member know and we will send to you in advance. just in case.
Ask ahead of time what types of payment are accepted. If you’re missing any vaccines, the doctor may provide them during the appointment or let you return with documentation from another provider. After the exam, the civil surgeon will complete your I-693 form, seal it in an envelope, and hand it to you. Request a copy of the exam for your records and for your attorney to review before submitted to USCIS. Do NOT open or alter the sealed envelope – USCIS will reject it if tampered with.
